As a touchstone, it is interesting to see how this was approached on the XS1100.

Big flange and a two piece bolt / washer combo. Also not a high strength fastener.

I'm wondering if some of the 2nd gear issues are related to loosening of this fastener and slop (engineering term) in the middle drive axle (layshaft)?
